Register with us When registering with MLIE: Read through the applicable Registration document and ensure that you understand all that it entails. If there is any aspect you do not understand please inquire with one of our consultants. National Candidate Registration Document (Word document) International Registration Document (Word document) Please send the following documents to us: (After establishing that you understand and agree with the Registration form.) 1. Completed registration form (initial all pages and fax back all the pages) 2. Copy of ID (clear copy please) 3. Copy of HPCSA registration certificate & card 4. Copy of updated CV 5. Copy of written references (3 if possible) 6. Copy of Certificates (Medical Diplomas/Courses done ATLS, ACLS, PALS and other) 7. Malpractice Insurance number! (Please make sure to cover yourself) 8. Please complete all Tax Information on registration form this is compulsory If you are not registered, please inform us so we can help you with the registration process. When registering as a client with MLIE: Please complete the Client Registration form below. Ensure that you give special attention to the following: 1. Be sure to initial each page, complete the last page and fax the whole document to us. 2. Indicate who is responsible for Payment (Name Surname, Tel, Fax, email) 3.
